Submit Handbook Forms and US Lacrosse Membership form with your $50 non refundable deposist to the Clippard Family YMCA. 

(Checks made out to the Clippard Family YMCA please).  

ALL REGISTRATIONS ARE DUE BY December 16, 2011 FOR THE 2012 SEASON!!!

A $50 non refundable deposit MUST be submitted by December 16, 2011 with the balance due by March 1, 2012.
